This one had plenty of faults, but for some reason I enjoyed it anyway.
Seems to be happening more, but the dead doctor was a repeat actor from about 15 years ago - he was hotel security or something similar at the place where Jenny got kidnapped by the brother of the guy Ziva murdered in the elevator at HQ. It was easy to tell the blood on the front of his clothes wasn't his own because it was red and not green.
Real people don't lie on the table for simulation exercises - there are incredibly advanced mannequins these days that have pulses, heart and lung sounds, eyes that react to light, etc. The lady doctor would have been in a separate control room controlling the reactions of the mannequin.
The agents go in pairs everywhere, except when Parker goes to visit the lady doctor. Only reason is to create the kidnap situation. If someone else had been there, even without doing a shootout, the other agent could have tailed the van. I recognized the Spies Like Us name right away, even if it was a combination of two names from the movie. Also worthy of note that the movie was frequently referenced in various ways on the TV show Chuck, where Gary Cole appears two or three times as the father of the female lead, who happens to be a career grifter. Parker acting all doctor-like after the rescue reminded me of a MASH episode where most of the camp had the flu and Radar was recruited to assist in surgery, passed out, then acted like Hawkeye's personal physician at the end of the episode. And yes, it was super obvious that Torres' back crack was going to cure everything, but surprisingly an internet search on 'Emmett Trowbridge' does not instantly pull up matches for Spies Like Us.

Not one of my favorite episodes but kinda fun. Parker is a good character and Gary Cole does him well. We'll see if the lady doctor shows up again.

It was easy to tell the blood on the front of his clothes wasn't his own because it was red and not green.
They killed the Changeling, that's why.

Ok this is a nitpick: The way the blonde doctor was speaking annoyed me. Is that an actress thing and does it all the time, or was it supposed to be a character thing?

Could you be a bit more specific? Was it her tone of voice, choice of words, or something else? The writer definitely felt that doctors are egotistical (must have met one), but this one seemed to be written as self-aware of the ego.

Like I said a nitpick. It was like she was surprised (?)by the words coming out of her mouth.

First of all...TUVOK! Too bad Tim Russ was killed off so quickly. Oh well. Still it was nice seeing him again.
Some of you have already mentioned some of the issues with it. I'll just put in a couple of more. Always hate it when one of our good guys allows himself to be kidnapped. And this one was set up so stupidly. You should always go in pairs, especially when Alden had a stiff neck and could barely function. Heck I'm not even sure he would be allowed in the field considering how restrictive his movement was. And that was pretty ballsy of the kidnappers to do it inside the hospital during broad daylight. Could have easily set off security and then been trapped inside and not be able to escape with the doctor.
And in the final shootout scene, there just happened to be the syringe with the drug right there for Parker to stab/shoot the drug into that dude's neck. And that it would act so quickly and drop him. To be fair though, I have no idea what that particular drug is or how fast it can stop someone's heart.
I did like the detective work on figuring how and where a OR could be set up. It was a bit of a longshot, sure. But I could see where it could be plausible. Though if he had that kind of $$, I'd imagine he would set up shop in some country without an extradition treaty with the US. But perhaps it would have been harder to source everything there.
I also liked the banter between Parker and the doctor (who's name escapes my feeble brain matter). Definitely looks like something is going on between them. Be interesting to see if she comes back. And I like Gary Cole and he shines in some of these Parker-centric episodes.
One thing I don't buy for a second though is Parker/Torres being so great at their 2 on 2 basketball that they are playing for the championship. Competition must be pretty bad for them to advance that far. Sorry....just doesn't seem believable in the slightest for me.
Still... I found it enjoyable enough. Even with some of it's problems, I'll give it a B-. Then again, sometimes I am just easily entertained.

Adding on to what I said previously about the teaching- they wouldn't be doing that in the hospital proper, but in a simulation lab. Seems unlikely the lab would have a biohazzard waste pickup. And absolutely not to the injection taking the guy down so fast. Might work as fast as 30 seconds if properly injected intravenously, but into the side of the neck like that doesn't even guarantee it was intramuscular, which would probably take more like 2 minutes. But, the instant drug has been a trope of TV and movies for decades, just like the team doing everything independently instead of calling someone closer or bringing in a SWAT team when going into a situation where there are likely to be a lot of people with guns. I figured the tournament was probably just with the gym or park district and was probably all guys who are 40+. On a side note, I think Tim Russ should have been used in an investigative role. He's good at combing through evidence:
